metapps.jar is no longer a valid product. Download the IDV JAR files from: ftp://ftp.unidata.ucar.edu/pub/idv/IDV2.0/idv_jars_2.0.zip and put the idv.jar from that ZIP file into your classpath. The relevant class is ucar.visad.ShapefileForm. Note this only works for shapefiles with lat/lon point forming lines, not UTM or point data.
