[IDV #GXD-384297]: supporting form for decoded/gem/surface

Thank you for your support request. I was able to successfully open
the data in the IDV with the following steps.

In a web browser, I navigated to
http://motherlode.ucar.edu/repository. I went to Data --> IDD Data -->
decoded --> gempak --> surface.

There I clicked on the small arrow next to the "GEMPAK File" link.
And I clicked on "All Links". Now click on the "OpenDAP" link.  Copy
and paste that URL into the IDV. In particular, in the IDV Dashboard
go to the "URLs" tab. Paste the URL in the textbox. Most importantly
select "netCDF/GEMPAK Point Data files" as the Data Source Type.
