that would be it!  comment out the entry in pqact.gempak_nwx and execute the 
command 'ldmadmin pqactHUP' and we'll likely see this problem resolved.  I 
don't see duplicate entries for dcmsfc, and the log isn't showing multiple 
processes trying to read/write to/from the same file, which makes me suspect 
your disk might be getting full?  check on this and let me know.
