You are correct that the CAL block is needed to convert the digital counts
to calibrated values _if_ the image is not already calibrated.  For instance,
the images sent in the Unidata-Wisconsin datasetream are already precalibrated.
The end user (you) do not need to worry about doing the calibration.

Also, for RAW images, the calibration block is contained in the AREA file
holding the image.  McIDAS commands use that calibration information to output
calibrated data values.

By the way, in addition to or replacement for AXFORM, you may want to 
investigate
use of the IMGPROBE command.
